PAYMENT AUTHORIZATION AND INFORMATION STORAGE
- I authorize HSLDA Action to charge my indicated form of payment immediately.
- Payments can only be made by eCheck / direct debit or by credit/debit card.
- HSLDA Action will securely store my payment information for recurring charges.
ANNUAL AUTO-RENEW
By submitting my membership application, I agree to the following:
- My membership with HSLDA Action is a 12-month term. HSLDA Action will auto-renew my membership each year.
- HSLDA Action will automatically collect payment on or close to my membership anniversary each year.
- If paying by eCheck / direct debit, I will be charged a $20 service fee if my payment is returned.
- Payment information updates must be received by HSLDA Action a minimum of two business days prior to my scheduled billing date and must include my HSLDA Action member number.

To cancel my automatic membership renewal, I will contact HSLDA Action to request cancellation and include my name and member number in any such communication. I must contact HSLDA Action at least four business days before my scheduled payment to
guarantee that payment is stopped. HSLDA Action is not obligated to refund my payment if I have not followed this cancellation procedure.
- Any promotional or discounted pricing is only available with the initial annual auto-renew payment. Future annual renewal payments are subject to membership prices then in effect.
ABOUT MY MEMBERSHIP

I may cancel my membership up to seven business days after membership activation or renewal and receive a full refund. Cancellation request must include my name and application ID or member number. HSLDA Action does not guarantee a refund if I cancel
my membership after seven business days.
- My membership is valid from the date my application is submitted.
- HSLDA Action reserves the right to revoke membership if any information has been misrepresented.
- Membership dues are not tax deductible.
- Membership is not transferable.
- HSLDA Action does not provide any legal representation.
HSLDA Action reserves the right to change these terms at any time.
